I recently moved to Eureka, CA from a state that made even giving patients information or referrals for abortion a felony. Part of the reason I picked this location is the extremely low religiosity. But in a city that's only 21% religious, 7.5% Catholic, the only hospital in the city is... Catholic. They also own most of the specialist offices and many primary care offices.
So we've got urologists not allowed to do vasectomies, etc.
I am looking into getting primary care in Redding, 3 hrs away, because of the waiting list here (yes, including for physicians). There's a large group there called Mercy Medical owned by Dignity, which sounds Catholic but doesn't say it on their website. Took a little digging to find out that yes, they use Catholic rules for services offered.
Physicians coming here have a difficult time competing with large employers, who are heavily Catholic.
I know the history of Catholic hospitals in the US, and I know the unfortunate way freedom of religion has been interpreted in the US. Imo it should have been freedom to believe/think as you wish but no loopholes on action. But short of a new Constitutional amendment, does anyone have ideas that might hold up in court to restrict these churches from imposing their beliefs on medical workers and patients? There seems to be very little choice in this area of shortage.

Hi fellow med students and Doc’s, have had a thought of mine for a while.
Have noticed a few people (both in my wider social group and at university) who are generally polite around me, talking behind my back in a negative manner that I’m arrogant, conceited, know-it-all etc. A lot of the time these groups who have been talking negative about me have been groups of girls (I’m a male) and they’ve tried to turn my friends against me (which hasn’t worked since they have told me). I have always tried to be cordial with everyone however.
This sort of gossiping has coincided with myself doing decently well in medical school, doing well financially and being well-liked by senior Doctors in the hospital who teach us.
Being self-aware, I do admit I am a direct person who isn’t afraid to compliment as well as criticise ideas which may rub people the wrong way. I also generally don’t smile often and have a more neutral facial expression.
However no one has ever had the courage to openly criticise myself in public and instead these people have resorted to charactereputation destruction behind my back.
Was wondering what strategies do you guys have about people who gossip and talk behind your back and to what degree can I control this?

Results from the phase 2 study, being presented at the 2023 Consortium of Multiple Sclerosis Centers annual meeting, demonstrate that frexalimab significantly reduced disease activity in patients with relapsing forms of the disease. “Frexalimab has a unique mechanism of action, blocking the CD40/CD40L costimulatory pathway thought to regulate both adaptive and innate immune cell activation and function – a pathway that is pivotal in the pathogenesis of MS,” explained Gavin Giovannoni, chair of neurology, Blizard Institute, Barts and the London School of Medicine and Dentistry, Queen Mary University of London. ———————————————————————
(From Giovannoni’s blog) CD40 is a critical costimulatory signal that drives T-cell activation and is involved in B-cell and innate immune activation. As the CMSC has started and the embargo lifted, I can now tell you the phase 2 Frexalimab study results are strikingly positive. These results are the most exciting to emerge in MS in the last 12-24 months. …because I am convinced that EBV causes MS and that continued EBV latent-lytic cycling drives MS disease activity, these results need to be interpreted in the context of EBV. It is no surprise to evolutionary virologists that EBV has hijacked CD40 and the CD40-ligand signalling pathways. Latent membrane protein 1 (LMP1) mimics CD40 to augment the normal CD40 signalling pathway. This gives EBV-infected cells a survival advantage. Blocking the normal CD40 pathway with Frexalimab may be enough to kill EBV-infected B-cells or at least inhibit their functioning. Even if I am wrong about EBV and Frexalimab, there is no doubt the results of this study will send a buzz through the MS community, particularly the immunologists and genomics experts who predicted these results decades ago.
Could Frexalimab be doing more? Will Frexalimab put pwMS into long-term remission and mimic the stunning results we see with alemtuzumab or AHSCT? Blocking CD40-CD40L interactions should convert a T-cell activating signal into a tolerogenic signal and stop autoimmunity. This is why I am so excited about Frexalimab as a potential treatment for MS. ———————————————————————
The study randomised 129 patients to receive either higher or lower doses of frexalimab, or matching placebo, for 12 weeks. After week 12, patients receiving placebo switched to respective frexalimab arms and entered the open-label part B, which is currently ongoing.
Following 12 weeks of therapy, the number of new gadolinium-enhancing T1-lesions was reduced by 89% and 79% in the higher- and lower-dose treatment arms, respectively, compared with placebo, meeting the study’s primary endpoint. Additionally, both groups treated with frexalimab showed reductions in new or enlarging T2-lesions and total gadolinium-enhancing T1-lesions. Frexalimab was well-tolerated, the company said, and 97% of patients completed part A and continued to the open-label part B. Erik Wallström, global head of neurology development at Sanofi, said:
"...we are committed to growing our robust pipeline of MS therapies by exploring multiple treatment approaches with unique mechanism of actions that have the potential to slow or halt disability, which remains one of the greatest unmet medical needs in MS today.” Sanofi has said it plans to initiate pivotal trials in MS in 2024.

I used to rely on the all mac world website but they dont have the latest version. Some of their Adobe stuff like Photoshop and Illustrator files are damaged and cannot be installed.
Also did a few research on this group that lead to other free mac app site but they either never update their site or just plain died
Im desperate and looking for a source that I can get a whole suite, if not then just the most basic stuff like Illustrator, photoshop, InDesign, after Effects, media encoder, and Premiere.

I've (29f, 315lbs) been trying to loseweight for years now. My size has always been a struggle and I keep hitting those milestones of "I never want to hit x weight". I know there's a lot to do with diet, but even when my diet wasn't terrible I would only maintain, never lose.
I've been seeing a weight loss dr (who keeps pushing surgery which i don't feel like I've resorted to yet... and his expensive nutrition subscriptioni can't afford to join) but I've gained 10lbs in the 6 months I've been his patient. I don't feel like he's really doing anything to help. He just keeps saying he's trying to get me in weight loss meds then months later says I need to be part of his expensive nutrition subscription (which i haven't bothered with because the group classes all happen during my work day and that's really all it gets you) in order to get approved for those medications. I met with their nutritionalist but had technical difficulties and instead of letting me reschedule she just emailed me a bunch of info sheets that were very little use to me. For example, she sent me a diet list that very clearly states it is only a temporary "reset" diet but never went over how long to follow it etc. Just "have an email good luck!"
I've tried to follow that diet to the best of my abilities but I'm finding it very difficult because it is so restrictive. I live in a household of 3 (I live with my parents) with myself included, and we all have different dietary needs and preferences. We are very low income so we can only really afford to make one meal for everyone, but it's hard to work around so many preferences. My dad can't stand anything that grows from the ground, so meals must contain some form of meat. He also refuses to eat the same meat two nights in a row, and the diet they gave me is literally just fish and poultry. (With fish being very expensive in my area) My mom had multiple health issues herself, which means she has to watch out for certain foods as well. I also admit to being a picky eater, mostly when it comes to textures. I love the taste of things like steamed zucchini for example, but the texture makes me gag no matter what I do. I also can't get family to cooperate. I have then the list of items on the diet but still whenever I entrust them with doing the shopping they end up getting stuff that doesn't fit, and I can't afford to do all the shopping myself.
I tried lowering my portions, loading my plate with veggies rather than the meats. I've cut my soda and sugary drink intake by at least half. I know that's a big factor but quitting those cold turkey does not work for me. I find that if I force myself to only drink water, I don't drink anything at all. I've cut out almost all snacks, and when I do snack I try to limit it to fruits and veggies.
I also got an above ground pool and go swimming for an hour a day now. The weight dr said to avoid walking "because my knees" (I've never had knee issues) and to find alternative forms of exercise and this so far has been one of the few exercises I can do without getting migraines.
Somehow I've gained 10lbs. I know I make a lot of excuses and that's part of the problem, but I'm really at a loss for how to overcome that. I know a lot of weight loss is a mental thing but when my mental health is so far in the pits its hard to get over those mental hurdles. How can I get in the right mindset to improve things and stop making these excuses for myself? Everytime I get the motivation to try again, I just think about all the things stopping me and it becomes overwhelming again...I feel like I've ruined my life and I'm only making it worse the harder I try to make it better.

The Minnesota DFL voted Tuesday to ban Nasri Warsame, the Minneapolis City Council candidate whose supporters stormed the stage of a convention earlier this month, from ever seeking the party endorsement.
It also approved a change to its rules that opens the door to potentially banishing Warsame — and others in his campaign — from the party altogether.
More than 300 DFL Party activists logged on to a 6 p.m. virtual meeting of the party's Central Committee and debated well into the evening. Within the Somali community, opinions over Warsame vary, but there is widespread agreement that a ban, potentially for life, would be extreme — and could damage the DFL's relationship with one of its most loyal constituencies.
"Many Somali organizers, politicians, and community members including me have worked so hard for the last decades to include new voters in the process and the DFL Party," Mohamud Mohamed, a member of the party's Central Committee who was part of a lobbying effort pushing for something less than a lifetime ban, said in a statement.
Mohamed said the proposed ban is "silencing" the Somali community, and is "counterproductive to the DFL Party's values of inclusion and democratic process."
DFL Party Chair Ken Martin responded to that criticism at the beginning of Tuesday's meeting, saying he had spoken with numerous leaders and elders in the Somali community, and he said they supported taking strong action. "The Somali community condemns it even more vociferously than we do," Martin said of the convention chaos that prompted it all.
After the votes Tuesday, Martin said the DFL's action "sends a message that we will not tolerate this stuff in the future."
Convention chaos The hubbub is part of the fallout from the Minneapolis DFL's 10th Ward convention May 13, when first-time candidate Warsame challenged one-term City Council Member Aisha Chughtai for the DFL endorsement. After hours of drama and confusion over the intricacies of convention rules, a group of Warsame supporters
stormed the stage as Chughtai and her supporters were preparing to give a speech.
The ensuing chaos prompted the police to be called and resulted in
finger-pointing over who was at fault. Several sought medical attention.
A Minneapolis blogger captured much of the scene on video, which gained national attention. Soon the state party found itself needing to respond not only on behalf of those involved, but also to the spectacle, which resulted in a convention recess without anyone being endorsed.
In
the video, Warsame can be seen standing back as the fracas breaks out. Several minutes pass before he attempts to calm his supporters.
Days later, dozens of DFL leaders, meeting as the party's Executive Committee,
voted to sanction Warsame by barring him from ever receiving a DFL endorsement and discussed the possibility of banning him and some of his campaign workers from party activities.
Before any discipline could be meted out, the party's much larger Central Committee needed to ratify the policy that would allow Warsame to be sanctioned. That was the task of Tuesday's meeting.
Warsame, who has said he condemns what happened but planned to forge ahead with his campaign, did not respond to a request for comment Tuesday evening.
Somali sentiments The 10th Ward race has gained significant attention and sparked division among Minneapolis' Somali community. Some fault Warsame for his past crude criticism of the former president of Somalia, Mohamed Abdullahi Mohamed, also known as Farmaajo, while impassioned Warsame supporters are bent on their candidate winning the race.
State Rep. Mohamud Noor, one of the chairs of the legislative branch of the DFL Somali-American Caucus, said he hopes the DFL leaders and 10th Ward candidates will work together to ease the polarization.
"We have had some instances whereby it was solved by the DFL leadership by bringing people together," Noor said. "We don't want one group versus the other."
Osman Ahmed, former state director of the DFL and 10th Ward resident, accused the party of showing a double standard when handling misconduct. Ahmed, who ran for the state House in 2018 and whose 12-hour convention was canceled after a ruckus broke out and police were called, said he's seen worse.
"Chairs were even flying around and the party never bothered to intervene or hold any of the candidates accountable then," he said.
Yussuf Haji ran unsuccessfully for the Ninth Ward council seat two years ago, and now lives in Burnsville. Haji and others say the DFL should have had representatives from the Somali community at the convention to demystify the process to voters.
The party hired a Somali translator for the 10th Ward convention, but he left after party officials said he was threatened.
"The DFL is not perfect, and I know they're doing all they can," Haji said.
But Haji said barring Warsame from ever getting the DFL endorsement is an "extreme" punishment. "Maybe he should forfeit the DFL endorsement for this year and not be banned for life."
The founder of the DFL's Somali-American Caucus, Jamal Abdulahi, did not attend the convention but said he has since spoken with people who were there. He called the accusations that Warsame and his campaign manager misled voters "bogus."
"People are fed up with the narrative that Somalis are violent," Abdulahi said. "A lot of times what happens is newcomers get pushed around; it's just that this group wasn't going to be pushed around. They stood their ground."

| Cause I'm thinking the guy on the right is unambiguously correct. Cops choose to join without any promises from slimy recruiters and a lot of them are LARPers who specifically were either medically exempted, or didn't like the rules and regulations the military operates under. Plus they can leave anytime they want. This is America centric, bc I don't know anything about how police internationally join, and how much oversight they have. Plus the primary objective of police in capitalist societies is to enforce property rights, often over human rights. The military in America are often from the poorest regions of the country, I saw this first hand growing up. They're fed sweet lies by their recruiters all too often, in some other countries they're straight up conscripted. The US military operates mostly to preserve the interests of the nation same as any other military, but compared to some state militaries are kept under reasonably good control. And critically soldiers are just a tiny piece of the overall machine, politicians are the ones who send them, with input from the bourgeoisie. It's the old and wealthy that send the young and poor to die on their behalf. And this is going to be a hot take, but yes soldiers are exploited a lot more than cops, especially groups like the Russian military. Much as I hate Putin for starting the war in Ukraine I can't hate the troops waging it, every day stories come out about Russians sabotaging the war effort or outright defecting. All the while their command treats them like disposable tools in a way that'd make our refusal to care about vets after their term look positively humane. It's the same general concept for most militaries, even if that's an extreme example of soldiers being used to grease the wheels of their respective country's capitalist system with their blood. Has anybody noticed that medical students are nasty towards same-sex, attractive people? I’ve seen it with both women and the guys— the amount of negative gossip someone gets seems to be related to their physical attractiveness.
Some the stuff they say is absolutely terrible. A girl who was sexually assaulted and is coping poorly has been our class’s “crazy girl” for all 4 years of medical school. A guy who confided in a friend that he was molested as a child was ousted by said ‘friend’ and people spread rumors that he made the molestation story up completely— a claim he could not possibly defend himself against. I think I’m above average attractiveness and a couple spread rumors about me “going for 18 year olds” when I genuinely don’t know any. This same couple (the bf hates me) spent all of four years spreading rumors about me to whoever would listen— yes, this was an ugly couple.
The list goes on but I noticed these kind of things happen more often with attractive students. I’m wondering if the reverse halo effect is extra pronounced in this group or if this is a normal amount of gossip. I’m getting the idea that my medical school class might have been shittier than average.

I and my classmate Bri are both 20sF in our first semester of a medical program.
She had approached several students in the previous day of class to request a copy of a quiz that she claimed she was unable to access. I was thinking about giving it to her, but she didn't ask me directly. Later on that same day, I was in a study group and one of my classmates told me that Bri had not yet taken the quiz in question. In my eyes, that's cheating - or at least shows the intent to.
Later on that night she did message me and ask for my copy, and told me she was worried about failing an upcoming exam unless she had those answers. (My professor will sometimes reuse quiz questions on exams) I wanted to give her the benefit of the doubt that she was being honest, but I am also not going to just hand answers over, so I lied and said I was having technical troubles and gave her a list of the topics I'd covered for the quiz in question. (Yes, I can be conflict averse at times. Working on that.)
Today in class, she took the quiz while we had some time to review for the exam. I was not pleased that she'd tried to use me so emailed the instructor detailing what was said to me and what I did.
I'm now second-guessing myself and wondering if I overstepped. I lied and possibly put Bri's academic career in jeopardy, as well as possibly contributed to her failure instead of helping her. There's also the fact that I snitched, and some don't look favourably on that. On the other hand, I have to bust my butt to earn my grade and it's morally and ethically offensive to me to cheat, not to mention that my program has an honor code I need to follow.

Background:
I was an enlisted Airman who applied for and was accepted to the U.S. Air Force Academy under the LEAD Program back in 2018. The program is targeted for Airmen who meet all of the regular USAFA admissions requirements and have demonstrated the aptitude to become an officer to attend the Academy. There isn't a lot of information out there for people who are trying to do this program, so I've attempted to fill the niche I saw by making semi-regular Reddit posts on my experiences. If you're interested in the
LEAD program or if you just want a behind the scenes look at that enigma that is the Academy, read on. As always, feel free to hit me with any questions you have about the process or life here in general!
Firstie Year: Senior year of the Academy is referred to as "Firstie" year. You are given a blue beret to stand out, you march at the front of Squadron formations, Permanent Party constantly teases you with the fact that you'll be out of here soon. Firsties are really treated better. You start seeing what the world is going to look like when you're done with this place: you get your AFSC awarded, your first base, your problems start becoming real things like PCSing and not having orders as opposed to worrying about class projects and parades. Every day, I got to wake up and see that the number of days until graduation was getting to the point I could actually visualize it happening.
Jobs (not real ones): This is the year where you become officers within the Cadet Wing and are responsible for leading the various mission elements. At this point in a cadet's career, they can manage any manner of jobs in the Wing from an Assistant Squadron MWR Officer all the way up to the Wing Commander. Now, while ostensibly the cadets are in charge of the Wing, there's still quite a bit of Permanent Party oversight, and while cadets may try to make decisions and do things, it always has to be done with the approval of the Permanent Party, and one could see how this sometimes generates friction. This is a statement that could just as easily be a 10 page essay, so we'll leave it there. You have to have a job, but realistically some jobs do much more than others, and as a firstie you can make your job just about whatever you want to be. In this, USAFA is still very much a choose your own adventure book. I worked a Group job in my first semester and then did the same job in my Squadron for the second semester, all in all a pretty good experience.
Jobs (real ones): When classes get their actual AFSC's varies. USAFA's A1 has consistently tried to get cadets their jobs towards the end of Junior year. This has never happened. The Class of 2023 got theirs at the end of August. Now with jobs, the name of the game is "What is the pilot fallout?". Most (more than half) people who come here want to be pilots and we had something in the neighborhood of 500 pilots and everyone else was spread around. Some people got pilot that weren't medically qualified and have to push for waivers, some people got other jobs entirely that wanted pilot. I can now say definitively that if you're medically qualified and you give half a shit during your first three years, getting a pilot slot couldn't be easier. Anyways, after you get your job, you and your friends go out to the mountains or Denver and have a party.
Frustrations: Standard disclaimer: My views are my own and do not represent the Academy in any official way. I wouldn't be giving this place a fair shake if I didn't state some of the negative stuff. Firstie year is also where the politics of the Academy start to actually matter. I had quite a few frustrations both personally and professionally because of this place. There are some pretty serious opportunities that you can try to go for, and end up not getting them because of some backroom deal or the decisionmaker simply liked another candidate for it more than you. It's hard to try hard to get something, only to be denied and no good answer is given for why. But hey, that's life, and it's probably a useful thing to get cadets acquainted with the actual politicking of the real Air Force.
Being a Prior: Honestly, it's not an identity here this late into the game. More is still expected of the prior enlisted, and now that we're getting to the point where we're rejoining the real Air Force, plenty of the directs have questions about how things are and how they should interact with it. But at this point I've spent more time at the Academy then I did enlisted. I identify more with the people I hang out with, and while groups of those happen to be priors, just as many aren't. I still am more than happy to talk to people about the real Air Force, and I will lean into the "old man" jokes that are thrown at me, but now it's just a fun fact as opposed to Freshman year when it was the primary part of what people saw me as.
Conclusion: 1781 days ago, I drove onto the base for the first time to inprocess for the Prep School. I was scared; the Academy was an imposing place both physically and in reputation. Now, I can count the hours until I commission on one hand. Tomorrow, I get to shake Mr. Biden's hand after I walk across the stage. And the day after that, it was all just a bad dream. It was not, it was not, it was not easy, but really at the end of the day I don't think I would have chosen a different commissioning source. There's a tendency to think of the Academy as wasted years, I prefer to think of it as years lived more intensely. I have made the best friends of my life, I have had some of the most incredible experiences, good and bad, and whether I'm a better person or not for having done it, I feel like I will be a better Officer because of it, and I am glad I went through it. I am grateful for everyone who helped me along the way (and that's a long list of people), and hope I can live up to the expectations they have for me as an officer.
